Games Like Plague Inc: Take Over the Map, or Defend It
What games are like Plague Inc?
Updated October 2026.
The closest game to Plague Inc is Bio Inc. Redemption, where you build a disease inside one patient or play the medical team trying to save them. Infectonator 3: Apocalypse has the closest world map: you drop a zombie virus and spread it continent by continent before humans finish a cure. If you play only one game here, make it Rebel Inc: Escalation, where an insurgency spreads region by region and you are the one holding the country together.
Plague Inc: Evolved is the PC version of Ndemic Creations’ 2012 phone game: your pathogen starts in Patient Zero, you evolve it, and you have to end humanity before humanity finishes a cure. It is on Steam at $14.99 regular with 66,022 reviews, rated Very Positive (as of October 2026). Everything below keeps one of its two halves: you are the thing that spreads, or you are the thing trying to stop it.
A Plague Inc-style game is a strategy game where something spreads, whether a disease, a zombie horde, an insurgency or a machine’s control, and the other side races to contain it before it wins.
Which one should you play first?
Rebel Inc: Escalation is the best game on this list because it gives you both halves of Plague Inc: in the campaign you contain an insurgency as it spreads region by region, and in Vs Multiplayer you run an insurgency into another player’s region while defending your own. Bio Inc. Redemption is the pick if you want to design the disease yourself. Infection Free Zone is the pick if you want to survive an outbreak on the ground.

Which games let you create the disease?
Bio Inc. Redemption keeps Plague Inc’s core: you build an illness and pit it against medicine. DryGin Studios zooms the whole idea down to one human body. The Steam page lists more than 600 real diseases, symptoms, tests and treatments, and two campaigns: “Choose Death”, where you combine conditions to kill the patient, and “Choose Life”, where you are the diagnostician racing to cure them. It is the sequel to DryGin’s mobile game Bio Inc., and that original is now listed on Steam as Bio Inc. Classic, free, with a release date of October 14, 2026 (as of October 2026). Its page sums up the series in one line: upgrade the disease “before a team of highly motivated doctors find a cure and save him”.
Infectonator 3: Apocalypse is the closest to Plague Inc’s world map. Toge Productions has you drop a virus into an unsuspecting population, watch the zombies spread, and move continent to continent while humans deploy countermeasures and research a cure; if they finish it, the run is over. It is pixel art, fast, and has more than 35 zombie types and more than 200 levels, per its Steam page.
Zombie Night Terror is the puzzle version. You are the brain of a horde that walks forward until it falls into a trap, and you mutate zombies across 50 levels to get them to the survivors. The mutations do the job of Plague Inc’s upgrades, and the store page promises the same pushback: “The closer you get to total world extinction, the harder survivors will fight.”
Which games are like Plague Inc but war?
Rebel Inc: Escalation is the best “Plague Inc but war” game. Ndemic Creations, Plague Inc’s developer, built it on its earlier mobile game Rebel Inc, and the Steam page calls it a political/military strategic simulation, inspired by modern counter-insurgency and by Afghanistan. You balance civilian and military priorities to stabilise a war-torn country while an insurgency tries to seize power. It has 8 regions and 8 governors, each with abilities of its own, such as the Economist, who takes a whole year’s income in one go.
DEFCON is the other answer, and the bleakest game on this page. Introversion Software’s 2006 strategy game, inspired by the 1983 film WarGames, puts you in a bunker during a global nuclear war. You place silos, submarines and air defences, then trade strikes across a world map while a counter tracks the dead in real time. Its Steam page sums up the goal: “In nuclear Armageddon, everybody loses. You just have to lose the least.” Like Plague Inc, it is a world map you watch fill up.
Which games put you on the side fighting the spread?
Plague Inc: The Cure is the most direct answer to “Plague Inc, but I stop it”, and it is the one pick here that is not a separate game. It is an expansion for Plague Inc: Evolved, so you need the base game, and it costs $5.99 regular (as of October 2026). You run the global response: testing, masks, lockdowns, contact tracing and treatments, built with input from the WHO, CEPI and GOARN according to its Steam page.
Infection Free Zone is the best standalone pick if Plague Inc made you wonder what the outbreak looks like on the ground. Jutsu Games uses OpenStreetMap data, so you type in the name of any real city or town, including your own, and set up a base there. You adapt existing buildings to new uses, set up shelters, build farms and power houses, put up walls and towers, scavenge the old pharmacies and grocery stores, and fight off the hordes of infected that can hit the settlement at night. Researching a vaccine or a cure is the win condition, the same race as Plague Inc’s from the other side. It is still in Early Access on Steam (as of October 2026).
Creeper World 4 makes the spread itself the enemy, which is what makes it the strangest pick here. Knuckle Cracker’s Creeper is a fluid that floods across 3D terrain in waves, and you build an energy economy, take the high ground and push it back. It is the only game on this page where the spread is a physical thing you can watch fill a valley, and you are the one trying to hold it back.
Is there a game like Plague Inc where you play as an AI?
Rogue AI Simulator is the best one, and it has Plague Inc’s detection problem built in. You run a Department of Science facility and its human test subjects, and the store page says their suspicion of you never switches off. Act openly and you risk an early defensive response; play the obedient servant and you can reveal your real plan at the very end, the same trade Plague Inc asks you to make between spreading fast and staying unnoticed. The payoff is a takeover too: at least seven endings, from Global Thermonuclear War to Secret World Domination. Its idle spin-off, Rogue AI: Idle Domination, is covered with it in our list of games where you play as the AI.
Endgame: Singularity is one of two free picks, and it runs Plague Inc’s race from the hunted side. Its project site describes it as “a simulation of a true AI. Go from computer to computer, pursued by the entire world. Keep hidden, and you might have a chance.” Swap the pathogen for code and the cure for the world’s search, and the shape matches. It is open source, written in Python, and its latest release, v1.1, came out on GitHub on June 4, 2025. The other free pick, Universal Paperclips, is a browser game where an AI with one goal converts everything it reaches; it has a page of its own, games like Universal Paperclips.
Why is Spore on this list?
Spore is the loosest fit here, and the biggest game on the list by reviews, at 72,846 Steam reviews (as of October 2026). Maxis built it in five stages, Cell, Creature, Tribe, Civilization and Space, and you take one species from a single cell to a galaxy. It shares Plague Inc’s arc of something small evolving until it covers a planet, but you design a creature, and nobody races you to a cure. Play it if the evolution screen was your favourite part of Plague Inc. Skip it if the race was.
What did we leave off, and why?
Pandemic 2 is the 2008 browser game Plague Inc’s developer has named as an inspiration, along with the film Contagion. It was a Flash game, and Flash is gone from modern browsers, so it is hard to recommend as something to play tonight.
Plague Inc: The Cure stays out of the tables only because it is DLC for Plague Inc: Evolved. It is covered above, under fighting the spread.
Where our game fits
Our own game, Sam, We Talked About This, sits beside this list rather than on it. It shares Plague Inc’s shape, one small start that grows until it covers everything, but it is an incremental game, not a race against a cure. An incremental game is a game built around numbers that grow faster than you can spend them: you buy something that makes more of the resource, which buys more of that thing, until the scale changes and new systems appear. In Sam you scrape whatever the internet has, train text, image and video models on it, let the models pull in subscribers, and put the cash into more disks, bigger machines and a bigger crew, until AGI is waiting at the end. It is unreleased, due on Steam in Q1 2027, so there is nothing to point at yet beyond its Steam page. If you want an idle game you can play today, our list of games like Cookie Clicker has ten that are free to play on Steam.
