Games Like Two Point Hospital: 5 Management Sims
Updated August 3, 2026
If you want games like Two Point Hospital, start with the rest of the Two Point series, Campus (2022) and Museum (2025), then Project Hospital for a deeper medical sim, the 1997 original Theme Hospital that it descends from, and Let’s Build a Dungeon for a chaotic build-and-run twist. All five share the same loop: lay out rooms, hire staff, and keep a busy building ticking along while the money and the comedy pile up.
I make management games for a living, so this is the list I actually reach for when I want that specific Two Point itch scratched. Below is what each one does, whether it is out, and who made it.
What makes Two Point Hospital worth copying?
A management sim is a game where you run a system instead of a character: you lay out the space, place the people, spend money and time you never have enough of, then watch the system push back. Two Point Hospital (Two Point Studios, 2018) works because of a tight loop wrapped in a joke. You place rooms along a corridor, hire and train staff, watch queues form, and nudge a light economy back into the black, all while a deadpan PA system narrates the chaos. The building is the puzzle and the punchline at once. If you want a longer read on why that structure holds up, we broke it down in what makes a management sim fun. Every game below shares at least the room-placement-plus-staffing core; they diverge on tone and depth.
Which games are most like Two Point Hospital?
| Game | Studio | Released | Tone | What it keeps from Two Point Hospital |
|---|---|---|---|---|
| Two Point Campus | Two Point Studios | 2022 | Comic | Corridor-and-rooms building and staffing, with students in place of patients |
| Two Point Museum | Two Point Studios | March 4, 2025 | Comic | Room placement and guest routing, plus expeditions that bring back new exhibits |
| Project Hospital | Oxymoron Games | October 30, 2018 | Serious | The hospital-building premise, rebuilt with real diagnostic depth |
| Theme Hospital | Bullfrog | 1997 | Sardonic | The template the whole thing descends from, corridors and dark jokes included |
| Let’s Build a Dungeon | Springloaded | Unreleased, due 2026 | Chaotic | The manage-a-building idea, pointed at a game studio and the RPG it is making |
Two Point Campus (2022)
The most obvious next step is the same studio’s follow-up. Two Point Campus keeps the corridor-and-rooms grammar and the comic tone, but swaps the hospital for a university: you build lecture halls, dorms, and clubs, then shepherd students across academic years. If Two Point Hospital’s humor already landed for you, this is the safest recommendation on the page.
Two Point Museum (2025)
Two Point Studios’ newest entry launched on March 4, 2025 for PC, PS5, and Xbox, with a Switch 2 version following on October 28, 2025. You curate exhibits, send experts on expeditions to bring back new pieces, and route guests past your best displays. It is the freshest game here and the one to grab if you want the current-generation version of the formula.
Project Hospital (2018)
For players who found Two Point Hospital too breezy, Oxymoron Games’ Project Hospital (released October 30, 2018) is the serious cousin. It keeps the hospital-building premise but layers in real diagnostic depth: departments, medical equipment, and a mode where you play the doctor treating cases directly. Less cartoon, more simulation.
Theme Hospital (1997)
Two Point Hospital exists because of this one. Bullfrog’s Theme Hospital, from 1997, invented the sardonic hospital-builder template that Two Point Studios later revived; many of the Two Point team worked on the original. It shows its age, but playing it is the clearest way to see where the whole genre started. It is not listed on Steam, so the GOG store page is where to find it.
Let’s Build a Dungeon (in development)
Springloaded is now making Let’s Build a Dungeon, a game-development sim due in 2026 on PC and Xbox. You build a fantasy RPG inside the game and also run the studio and the world around it, which puts a creative-tools spin on the manage-a-building idea. A demo is available on Steam, and the game is still unreleased as of August 2026, so treat it as one to watch rather than one to buy yet.
Where does a cozy, comic take fit in?
Our own debut, Grandma Left Me a Building, sits next to this list rather than on it. It is a 2D apartment-management sim: you inherit a run-down building from your grandmother, take in odd tenants, keep mismatched neighbours from clashing, collect rent, and upgrade the place. It shares Two Point Hospital’s comic streak and its “build, staff, and balance the books” spine, but runs on a turn-based day loop with no time pressure, so you plan at your own pace.
The cast is where it leans weird: 30+ tenants across four social tiers, including an opera singer who bursts into an aria in the elevator and a philosopher who answers every question with “but why?”. Trait-based pairings mean the right neighbours bring calm and the wrong ones bring chaos, and the chaos is the comedy. It is unreleased, so no scores or sales to point at yet. If it sounds like your kind of building, you can wishlist it on Steam or read how we designed the world of Grandma Left Me a Building. We also put it side by side with Stardew and Two Point in cozy management games compared.
